You can tell us online or by phone.
You'll need their:
• account holder's name
• date of birth
• address
You’ll need your:
• name
• address
• your relationship to the deceased
It's useful to have their latest bill in case we need other information, such as their account number or last bill amount.
We don't need to see the death certificate.

You'll need the account number or BT telephone number (mobile or landline) to use the form.
